What a missed call really costs your clinic (and how to work out your own number)

Most clinic owners never count missed calls, so the loss stays invisible. Here is a simple 5-minute way to put a real number on it — using your own call log, not someone else's case study.

Ask a clinic owner how many patients they lost last month and they will usually say "not many". Ask how many calls went unanswered and the answer is almost always "I don't know".

That gap is the problem. A missed call doesn't show up anywhere. There is no empty chair with a sign on it saying this patient called you at 9:40 pm and then booked with the clinic down the road. The loss is real, but it is silent.

This post gives you a simple way to make it visible. You only need your phone's call log and 5 minutes.

Step 1 — Count the calls you didn't answer

Open the call history of the clinic number (or ask your phone provider for a monthly report) and count, for one normal week:

  • Calls that rang out and were never answered
  • Calls that came in while the line was busy
  • Calls after closing time and on your off day

Don't guess. Count. Most owners are surprised by the after-hours number alone.

Step 2 — Estimate how many were real patients

Not every missed call is a new patient. Some are suppliers, some are wrong numbers, some call back later. Be conservative.

A reasonable starting assumption is that about one in three missed calls was someone who wanted to book or ask about a visit. If you know your clinic better, use your own ratio.

Example (not a real clinic): 25 missed calls a week × 1/3 = roughly 8 patients a week who wanted to talk to you.

Step 3 — Estimate how many of them went elsewhere

When a patient can't reach you, some will try again. Many won't — especially at night, or when they are in pain and the next clinic on Google Maps picks up.

Again, be conservative. Assume half of those patients booked somewhere else.

Example: 8 patients × 1/2 = 4 patients a week lost.

Step 4 — Put a value on one patient

Use your average bill per visit — and if patients usually come back (dental, physio, follow-ups), use the value of the first few visits, not just one.

Example: if the first visit plus one follow-up is worth ৳3,000 to you, then 4 patients a week × ৳3,000 = ৳12,000 a week, or roughly ৳48,000 a month.

Your numbers will be different. That's the point — work them out with your own call log.

What usually causes the leak

In almost every clinic the reasons are the same:

1. Peak hours. The receptionist is checking a patient in and the phone rings twice more.

2. After hours. The clinic closes at 9 pm; patients search and call at 10 pm.

3. Off days and holidays. The phone simply isn't covered.

4. No callback system. Missed calls are seen the next morning, if at all, and nobody owns calling them back.

Ways to fix it (from cheapest to most complete)

  • A callback rule. Someone checks missed calls every 2 hours and calls back. Free, but it depends on discipline and still loses the after-hours patients.
  • An auto-SMS or WhatsApp reply to missed calls. "Sorry we missed you — reply with your name and preferred time." Cheap and catches a part of the leak.
  • An AI receptionist. A voice agent answers every call, 24/7, in Bangla or English, answers the common questions, books the appointment into your schedule and sends a WhatsApp confirmation. Urgent calls are passed to a staff member straight away.

The right choice depends on your number from Step 4. If the leak is ৳5,000 a month, a callback rule is enough. If it is ৳50,000, an AI receptionist usually pays for itself within the first month or two.
